Patient Information
Your first visit at Oconee Physical Therapy and Sports Rehabilitation will include completion of paperwork, a complete comprehensive evaluation by a licensed physical therapist, and initiation of a treatment plan including a home exercise program. You will be educated on your injury and how to modify your daily activities to help decrease your symptoms and increase your function.
If you would like to complete your paperwork at home prior to your visit, please feel free to download the Patient Information Form and the Past Medical History Form, complete them and bring them to your first visit. Please remember to bring your insurance card and the prescription from the physician. Remember to wear comfortable clothing so we can get to the body part that we are working on."
